Now implemented, by default, CAPTCHAs are triggered on the following events:

there are apparently several different options:


* If an anonymous user adds a link, discard that edit unless that user passes the captcha test. Even if he does pass the test, discard the edit if that link is on the blacklist.
* New user registration
* If an anonymous user makes any edit, discard that edit unless that user passes the captcha test.
* Anonymous edits that contain new external links
* If any user adds a link, discard that edit unless the user passes the captcha test.
* Brute-force password cracking
* ... and other options.
 
Please note that a properly-implemented CAPTCHA system uses plain text questions (not those garish eye-straining images) so that blind people can use them.
